100th Birthday

Use this form to submit a 100th birthday to the Record Searchlight by following these steps:

1. Fill out the 100th birthday form completely (below).

2. Use the form to select a color photograph of the person celebrating their 100th birthday from your computer. Since only head and shoulder views are used in the newspaper portrait photos are best. Please name the photo using the first and last name of the person celebrating their 100th birthday (example: John Smith would be johnsmith.jpg).

3. If you are unable to submit a photo due to technical reasons please mail the photo along with a print out of this completed form to the address listed below. Please include your mailing address if you would like the photo returned.
